What is the HCD Genesis Model?
HCD Genesis is a single-management, personalisation-first model that integrates everything needed to support someone at home following secure-unit discharge — and to prevent admission where risks are rising.
HomeCare Direct works in true partnership with the person and their family to design, recruit, and train a bespoke support team (including known family members, where agreed by commissioners). HCD is the legal employer for that team and provides clinical oversight, training to CQC standards, and 24-hour resilience so support is safe and sustainable.
In short: one accountable provider, one personalised plan, one selected team — all built around the person.

- People with complex autism and/or learning disabilities, often with associated mental-health needs.
